What types of information do we collect from you?
Personal Information is only collected with your knowledge (i.e., if you voluntarily provide it to us through the “Contact Us” function or other means available on this website) and will usually take the form of a request by you for information regarding our services.
We do not knowingly collect Personal Information on our website directly from children under the age of 13 and will destroy such information if we become aware that a child has disclosed such information to us without proper parental consent.

As directed by Douglass & Zook, Google will use this information in order to evaluate usage of our services, to make reports on website activities and/or to other such services as we may request. Any information about you collected by Google Analytics (or similar third party tools) will not be matched up with other data of Google (or the applicable third party).
Legal and Business Transfers
We may also disclose personally identifiable information if we are required to do so by law or if, in our good faith judgment, such action is reasonably necessary to comply with legal processes, to respond to any claims, or to protect the safety or rights of Forest Lawn, its partners, or the public.
Funeral planning is funded through the purchase of whole life insurance from National Guardian Life Insurance Company, Madison, WI (NGL). A qualified Forest Lawn Planning Advisor who has been both licensed by the California Department of Insurance and appointed as an agent of NGL can answer any questions. Forest Lawn Mortuary (licensed by the California Department of Consumer Affairs as Douglass & Zook Funeral and Cremation Services) is an agent of NGL. National Guardian Life Insurance Company is not affiliated with The Guardian Life Insurance Company of America, aka The Guardian or Guardian Life.
